Job Description

Position Title: Care Coordinator

Reports To: Care Coordinator Supervisor

Department: Care Coordinator Department

Hourly Rate: $23-$35 Per Hour

Summary:

Care Coordinator is responsible for client-facing tasks, conducting assessments, developing care plans, and ensuring clients receive appropriate support. This role manages high-risk complex cases and supervises care coordinators I and II. This role provides leadership in resolving escalations, ensuring compliance, and contributing to strategic improvements in care coordination.

Primary Responsibilities

  • Manage a caseload of high-complexity clients.

  • Conduct assessments, reassessments, and wellness checks to create and modify individualized care plans.

  • Supervise and mentor Care Coordinators I and II, providing support and performance feedback.

  • Handle escalations for complex or critical cases.

  • Coordinate services, including medical, social, and behavioral health support, as outlined in care plans.

  • Serve as the primary point of contact for clients, addressing concerns and providing updates.

  • Monitor team performance and client outcomes to ensure effective care delivery.

  • Collaborate with supervisors and leadership on operational improvements.

  • Communicate care needs and updates to Care Coordinator II for caregiver scheduling.

  • Document client interactions, care plans, and progress updates accurately and in compliance with HIPAA.

  • Report significant client changes and concerns to Care Coordinator Supervisor.

  • Maintain productivity standards and adhere to timelines for assessments and documentation.

  • Ability to utilize Divine Agape Health Care Agency’s chosen Care Management Software.

  • Coordinate with Intake Department and other relevant departments.

  • Create a supportive environment for the client.

  • Manage and Supervise Caregivers.

  • Adhere to company policy.

Qualifications

  • High School Diploma or GED (Required).

  • Associate’s or Bachelor’s degree in Healthcare Administration, Social Work, Nursing, or a related field (Preferred).

  • 1-2+ years of experience in care coordination, case management, or healthcare administration (Required).

  • Strong leadership, organizational, analytical skills.

  • Completion of ongoing training requirements (5 hours of annual training).

  • Successful completion of a background check and fingerprint clearance through the California Department of Justice.

  • Ability to pass TB Test

  • Permanent resident card (if applicable)

  • Employment authorization (if applicable)

  • Must be a US Citizen

  • Valid Driver’s license and reliable vehicle
